If you do not want to create the quiz yourself we are willing to input the information for you but we will require you to submit the information in a specific style.

• Patient Profile

  • Please submit a document including all patient information as well as a patient photo if desired. Include all additional documents you would like included as well

• Periodontal Chart

  • Please submit a screenshot of the periodontal chart

• FMX (Location and Label for x-rays)

  • Please include all x-rays with the title of each x-ray including the letter associated with the appropriate location. please see this file to find the appropriate label

• Panoramic Radiograph

  • Please label this Panoramic or Pano

• Intra oral photos (Location and Label for photo)

  • Please include all photos with the title of each photo including the letter associated with the appropriate location. please see this file to find the appropriate label

• Additional questions (Example Question Sheet)

  • For each additional question please include a file with 3 distinct and easy to interpret sections as seen below. We recommend an excel file (example) but will accept other forms if discussed with us prior

    • The Correct Answer or Answers if there are multiple

    • All possible incorrect answers
